6 to 8 sessions Required. Experienced Practice Nurse, with experience in Chronic disease management. Will work alongside our existing Practice Nurses & Healthcare assistants.
Additional information about practice:
Two Practices, Churchside & Scrabo View working together. Building currently under renovation to facilitate our new MDT Team. Both practices are Teaching Practices.
